Poor diet: I don't consider you overweight and you are probably under weight for your height and sex. One consultation with a dietician will teach you what your ideal weight is and what is a proper diet for your age and height and level of physical activities. A physical trainer will instruct you on proper exercises for your age. These measures will enable you to lose fat and build muscle appropriately.
