Tropical disease?: Complex issue that needs medical attention asap. Could be a tropical infectious disease from a variety of bacteria or parasites. Or die thug as simple as a food borne illness. If the local medical care is inadequate, you'll have to go to either a larger city, or back to the US. There's probably some expats there that could help find a good medical facility.
