Unsure: Maybe stool was sent for culture? Maybe it was tested for blood and a negative result is reassuring that the source was not internal. Perhaps most commonly, 5-year-olds will avoid stooling because of discomfort. Then the larger, harder stools that result can damage the rectum upon passage. Encourage hydration (water, not juice) and adequate fiber intake to help soften things.
