PT: For these types of lesions I recommend a good course of physical therapy. If you are having a lot of pain then a cortisone shot can help. Surgery is most likely not needed.

Ortho, PT: You could start with physical therapy, however some tears of the rotator cuff require orthopedic surgery. I would refer you to orthopedics. In the meantime do rotator cuff strengthening exercises.

T Rotator cuff: There are three tendons which comprise the rotator cuff. Most commonly the supraspinatus or infraspinatus are injured . Treatment begins with NSAIDs , , P. t. . Activity restrictions. If little improvement in 4-6 weeks add one cortisone shot to the same regimen. Again if not resolved or improved surgery can be considered,
