PT: For these types of lesions I recommend a good course of physical therapy. If you are having a lot of pain then a cortisone shot can help. Surgery is most likely not needed.

Ortho, PT: You could start with physical therapy, however some tears of the rotator cuff require orthopedic surgery. I would refer you to orthopedics. In the meantime do rotator cuff strengthening exercises.
