Both: It is usually decided by the surgeon in consultation with the anesthesiologist. In some cases one or the other may feel there is a strong need. The longer the surgery, the more complicated the surgery, and the more pre-existing medical problems, the higher the need for the catheter.

It depends: It is usually the surgeon. They may consult the anesthesiologist if there are questions. Most anesthesiologist will require a Foley catheter if they place an epidural for post-op pain relief.
