Possible answers: One could always consider basilar migraine, but age a little older than usual. Raeder's paratrigeminal neuralgia could do this, a vasculitis or even a paraneoplastic meningeal process could be considered. Might search in retro-orbital area. CAT is useful if obvious large lesion, but MRI would be more reassuring.
