I have microscopic blood in my urine now for a year. i also have flank pain. ct was normal. what can cause this?

Multiple causes: low grade infection, bladder inflammation, hormonal issues, vigorous sexual activity/manipulation. bladder A-V malformation. I would suggest a urology consultation since this is now a longstanding condition and may require cystoscopy.
