Variable significanc: It depends on how large the fistulas are. A small coronary fistula can be without major effect. However, I have seen large coronary fistulas that require surgical or catheter repair. I suppose "bilateral" means both left and right coronary arteries have connections to either a coronary vein / atrium or other venous structure.
