Hard to tell: B/c of how the nerves to the pelvic organs work, it's hard to tell if the cervix is really dilating (it shouldn't dilate much at all, just enough to let blood or clots out), but the pain can definitely "refer" to the vagina and buttocks/rectum. Ibuprofen usually helps, and if this is a regular problem with all your periods, birth control pills may help decrease this pain a lot.
