No surgery guarantee: There is always a chance it won't heal, but if done well, the odds are in your favor assuming you don't smoke. Recovery can be 6 mos to a year before 100%

Varies incredibly: depends on the size of the gap healing has to fill and the ability to heal it. diabetics, weak bones, low vitamin D or calcium, nerve damage in the area, bad circulation all make it rough to heal. AND if it didn't already heal that is an indicator of problems.
