I have been taking ketoconazole orally for a month. the majority of lesions have gone but one will not go away. ringworm superinfected by bacteria?

It is possible: that you have a lesion that is not well vascularized and the medication is just not reaching it well -- or like you said it may have some bacterial infection. I would use ketoconazole cream on that lesion twice daily, and if it doesn't help or get worse bring it to the healthcare provider to determine if another cream or ointment is needed.
