Concerning : It is concerning. Do you have a history of cancer? There are other possible causes but it needs explored. Lytic lesions of the bone are often metastatic (from cancer some where else in the body that seeds to the bone). Have you had regular preventive health exams such as prostate exam and colonoscopy when due? Really should be further evaluated by your doc.

Based on the: description it could possibly be malignant, but there are potential benign causes as well. Follow up with your doctor about next steps.
