What does it mean when you breathe & your chest hurts to your back while sick?

Pleuritis: Likely, this is an inflammation of the lining of the lung. There could be other explanations but it is not clear what you mean 'while sick'. Other such things can cause this including pneumonia and pulmonary embolus.
