Ankle pain: Recurrent, intermittent ankle swelling and pain can be from arthritis. Would recommend ankle xrays to rule out arthritis.

Maybe Gouch.: "Swells up from time to time and has acute pain", if you add "redness and warm", it sounds like a perfect description of gout. In fact, redness and warmth may not even be as severe in in ankle attacks.

CONTRADICTS: "Swells from time to time" implies something ongoing and chronic vs. "acute" pain; sudden onset. Need a better history.
