Difficult to say...: ... without actually examining you. If the swelling is reduced, then most likely no incision is necessary. Regarding treatment of the failing root canal, ask your treating dentist.

See a dentist ASAP: Antibiotics are rarely required and should only be used as an adjunct to dental treatment for infection since they may complicate further outcome. Take the NSAID OTC or Tylenol (acetaminophen) to control pain and see a dentist ASAP.
