Subtle: Right sided heart failure would have to be fairly severe and sometimes longstanding to show significant changes on a standard CXR. Enlargement of the right ventricle and atrium are much better seen with an echo than a CXR. A skilled radiologist might detect some changes earlier than most but overall a standard chest X-ray is not a great way to look for or prove right sided heart failure.
