Probably: Especially if the nanny is on anti-HIV drugs. HIV is usually transmitted through blood -to-blood contact and thus I'd not expect this to be an event happening in child care. On the other hand, if the nanny contracted HIV through illicit drug use via a contaminated needle. you will need to carefully evaluated this aspect.
