TMJ: You have covered most of the important areas. This makes me wonder if you have some "minor" yet painful problem . If you have TMJ this might feel like that. You did not say where on the side your head hurts. It also could be there if you have dentures that do not fit right. An otitis externa can also feel this way. Instead of the ER see your primary care doc or get one.
