ENT: i would suggest further evaluation by ENT doctor; your symptoms are not typical for post-concussive syndrome and may indicate a problem previously undiagnosed. Make the appointment very soon

TMJ: Suspect at time of head injury, you also injured jaw and now have TMJ symptoms. A dentist could evaluate, and an oral surgeon might provide valuable help.
