Only very small ones: Small gall stones that can pass thru the cystic duct into common duct are the only ones that pass spontaneously. The common larger stones drop to the ampulla of the GB where they can impact on the cystic artery feeding the GB to initiate inflammation and necrosis. These stones can only be removed by surgery either, cholecystectomy, or opening the GB to remove stones, the cholecystostomy
