Appendix pain?: I hope it's not appendicitis? Considering your underlying CKD stage 4 disease, I suggest you need to be evaluated ASAP by a MD with appropriate blood and imaging study. Management depends upon specific etiology behind the symptoms.

Go to the ER: I am presuming you mean chronic kidney disease stage four. An infected appendix should not cause neurologic/ dermatologic symptoms in all extremities or pain on entire right side of body. That is more suggestive of a systemic problem that may or may not have anything to do with appendix or kidney disease. Appendicitis is serious and SX you describe may be. Need to be seen now to determine cause.
