Get better antibioti: Zithromax is fairly good for respiratory organisms but will get better cure rates with augmentin (amoxicillin and clavulanate). That Azithromycin isn't stopping the monthly sore throat may mean the organism is, partly resistant. May benefit from throat culture. If the fever is high and there are white sores with red rims in the mouth may have marshall syndrome which is often cured by t and a.

Culture/sensitivity: Zithromax, while a good spectrum antibiotic, especially for upper respiratory infection, may be unsuited to your pathogen. A throat culture and sensitivity test to find the proper antibiotic is really the best route. Recurrent throat infections like yours are best evaluated by an ENT specialist, and perhaps your tonsils and adenoids could be the culprit and require removal. Best of luck.
