Possibly both!: The blood clot forms from the base of the socket; additional clot may form from the surface. Sometimes the blood clot in the socket breaks loose, exposing the bone in the socket. This is called a dry socket and can be painful. If this happens your dentist will likely place a sedative dressing over the socket to protect it until a new clot forms.
