Is the incidence of rhd in the u.s. really high enough to mandate rapid strep swab/culture instead of just relying on the centor score telemed visit?

More than RHD: Streptococcal sore throat has more than rheumatic fever as a potential complication. The other serious ones being scarlet fever, post-streptococcal glomerulo-nephritis, quinzy, otitis media, and spread to others in contact with the patient.
