See doctor now: Take him to his pediatrician, an urgent care or emergency now. Do not wait, have him see a doctor as soon as possible.

Fever/swelling: A fever is an indicator of infection. I am not sure what is swollen? Is it his head or in front of his ears? Have him seen to help with diagnosis. All the best.

Fever: I'm not sure why the temples are swollen, try to control to control the fever with tylenol (acetaminophen) or ibuprofen & luke warm bathing & take him to his pediatrician. Good luck.
