See allergist: If you have nasal polyps with chronic sinusitis, you may need more than levofloxaxin and methyprednisolone. Consult an allergist for both the nasal and asthma issues. An ENT consult may also be needed later.

Sinus infection meds: Levofloxacin and methylprednisolone together are usually effective for sinus infections. Often Augmentin (amoxicillin and clavulanate) is tried first and levofloxacin is reverted for treatment failures or penicillin-allergic folks. When you have polyps, it is much more difficult to clear sinus infections and surgery for the polyps may have to be pursued before the infection is controlled. High dose steroids may help. Irrigate

Yes and no: They are often used but studies suggest that antibiotics have little value in treating sinusitis.

Therapy : In complicated sinusitis based on imaging such therapy may be an option but in most patients this may not be needed. The choice of therapy should be based on imaging symptoms and your MD input. If severe symptoms then see an ENT input.

Mgt/workup: If you have nasal polyps, aspirin sensitivity and Cystic Fibrosis should also be considered. Cultures of the sinus/nasal drainage can help to guide the therapy.
