This is serious: If your mother has a fasting BG of over 400 and another BG of over 500 SHE NEEDS TO GO BACK ON INSULIN RIGHT AWAY and see her regular doctor as soon as possible so she doesn't end up in the hospital. As for the TSH being low, a lot depends on whether or not her T4 level (or free T4) is high and if it is high she needs to be seen by an endocrinologist soon.

542 and 400+: Mom must go immediately to the nearest Emergency facility and more than likely will be admitted to hospital to control the diabetes. Wish her well!
