See a podiatrist: There is probably something left inside your foot that is irritating the tissue and has become infected. It has to come out!

See a physician: Go see a foot / ankle specialist. Your doctor needs to evaluate the hole in your foot, and determine if you have an infection. Foreign bodies are sometimes seen on x-ray specially if it's metal. Most likely your physician will need to do surgery to take it out.
