Possibly a hernia: one of the most common causes of pain in the lower abdomen and groin area is a hernia. You may or may not see a bulge with the hernia. Most often the symptoms are worse with heavy lifting or strenuous activity. Usually a good physical exam by a surgeon is diagnostic.
