I have ect sessions twice every week, except one week in the middle where i'm only scheduled for one. does this reduce overall efficacy of the ect?

Overall, no.: It may mean that on that particular week the one treatment merely maintains what was done the prior week, and doesn't add an additional effect itself. But the next week's two treatments would add effect as usual.
