IV line: IV access is usually established on 2 parameters: ease of access of the vein through inspection, palpation and experience/ training of the phlebotomist/ nurse, etc.

You get what you get: In the world of IV starting & use the general rule is to start down stream and work back upstream if needed. The hand is fine if you can seat one that works, but if it is lost you can move upstream (arm). Going downstream seldom works. When larger caliber IV's are used it is often started in larger veins upstream, since the hand may not tolerate them.
