Hard to say: He could be developing an infection - perhaps kidney or flu like infection. He could also be having a heart problem. Some people have decreased blood flow to the heart muscle and do not experience pain but rather symptoms such as sweating, nausea, dizziness. Given the history of very high BP I would have him checked at the ER.
