Surgeon preference: Adenoids may cause recurrent infection or nasal obstruction. Medications have limits. The traditional way of removing adenoids was with a curette - scraping them. More recently there are newer methods which reduce blood loss and operative time. Shavers that cut and suction the adenoids are preferred by some surgeons; my preference is to use an instrument that removes the tissue as it is cauterized.
