Fades over time: When mastectomy performed, the incision previously used, the Halsted incision is not used. Rather a transverse incision across chest is employed that when done carefully, leaves minimal scar that slowly fades until after 1 yr. may be hard to notice. This holds true with tram flaps as well as with free flaps that are now used for reconstruction unless patient is keloid former.

Yes and yes: There's no such thing as a scar that isn't permanent (except those that are made in utero), but many fade considerably and some to the point that a patient may feel it disappeared. Scars are advantageously planned by plastic surgeons so as to usually be felt by the patient not to be a problem. Scars do tend to be more noticeable in the teen years. See only an ASPS/ASAPS plastic surgeon.
