Hard to know: Pain and swelling is usually related to some type of trauma or infection. If it is truly random, then one would have to suspect angioedema/hives, which occurs both due to hereditary causes or to allergy or some other trigger for mast cell release. Medication side effect is common. Progression to swelling of the throat can be deadly because of an inability to breath. Consider having this evaluated.
