Colon cancer often: spreads to the liver,which could cause a rise in bilirubin.A single lesion in the liver has the possibility of removal,if the anatomy is favorable.

Elevated bili: Bilirubin goes up due to two possible causes in this situation. First would be some sort of blockage in the biliary drainage, which might be due to physical obstruction by a metastasis. The second would be due to incompatible blood that is being hemolyzed by an immune reaction. If no blood tx hx, then mets are likely cause.

Duct obstruction: When colon cancer metastasizes to an area resulting in bilirubin elevation, this is secondary to liver mets Single or multiple metastatic faci are unrelated to bilirubin elevation. This occurs when bile formed normally tries to pass thru the duct system to the GB and then to the ampulaa of Vater. A porta hepatis lesion will block flow of bile and cause regurgitation into the circulation.
