Get checked: You didn't say whether you have other associated respiratory conditions like asthma, chronic lung disease, sinusitis or even whether or not you're a smoker or have GERD. With such a persistent cough & increased effort with breathing, you should get checked for pneumonia. Hope that helps.

Infection: Sounds like you may have an upper respiratory infection that is aggravating underlying reactive airway disease. I would seek the attention of a primary care physician for further eval and treatment.
