Culture: I am not sure what urinary stream you mean but a clean catch which is sent for a urine culture is sent from the midstream if possible and if not possible must clean the outside of the genital prior to voiding.

Midstream specimen: Almost any urine specimen, whether for urinalysis or culture for UTI (with Klebsiella or anything else), should be a clean catch, midstream specimen. The main exception is STD testing (gonorrhea, chlamydia), which is best on first ounce (30 ml) of voided urine.
