Drill her the truth : Your care providers are not there to judge you , but to help you . They should know everything about you . Especially of you suffer from depression your midwife will need to know to help you better and be better prepared.

So?: She's trusting her life & her unborn child to the midwife but she doesn't trust the woman to be able to handle self-injury scars? Maybe she has the wrong midwife. More likely, the scars are recent and she's worried the midwife will correctly deduce that she has a mental illness which is not being treated and be concerned. Any reasonable person would be. She needs help not denial.
