It is possible: Abnormal Lymph node enlargement commonly results from infection, immune response or other causes. Therefore, the direct clinical examination is essential for diagnosis. See your PCP first.

No and yes: epid idymitis or inflammation of spermatic cord is a localized process, and even if nodes were to be involved, they would be local and not involve nodes in the region of the neck. Infection is not the final dnominator of enlarged nodes. They can occur with inflammatory processes such as Beck sarcoid where pus or infection in not a noted featue.
