Surgery may be best: Without knowing all of the details, your implant isn't likely to "heal" any more than what is has after 5 months. With significant pain at this point, a surgery is the treatment most likely to resolve it.

Depends: It should be fully integrated by now, so removal will be traumatic to the area. Sometimes, we are able to "back it out" or unscrew it. Other times we must trephine or "core" it out (like coring an apple). If you have had some lessening of symptoms and some return of feeling then you might wait and see. Nerve tissue takes the longest to repair. If no change, then consider removal. Best of luck.
