Need to see surgeon: elevated WBC does not mean acute appendicitis normal CT scan does not ruled out acute appendicitis. the most important sign of acute appendicitis is a localized tenderness ,with a local peritoneal irritation sign. persistent pain in RLQ need a surgeon to evaluate. good luck
