Fungus: I'd be suspicious that you have a superficial fungal infection. It doubt it's related to an ingrown toenail (with removal?) last year. There are many over the counter treatments - look for an antifungal cream like clotrimazole or lamisil (terbinafine).

Nerve endings.: Either it is a fungal infection, which I think you should be able to see, or a phenomena known as post injection site itching. The itching doesn't usually last as long as you have had it, but it sounds about right. Try applying a combination of topical anti-fungal and topical cortisone creams. If it is caused by either condition, you should find relief.
