Observation: if you are 40 years old, have no family history of ovarian or breast cancer, have regular periods and can go back to your gynecologist for another ultrasound in a few months, it might be worthwhile because a simple cyst that is clear and less than 8cm is unlikely to cause problems and will often resolve itself. if you are noticing changes in appetite and weight, or pain, you might need surgery.
