Would go to ER: A kidney infection, or pyelonephritis, is also often associated with high spiking fevers as well as flank pain. If you have been diagnosed with this, a urine culture should have been done which can help to guide an appropriate antibiotic. If the pain is worsening and you are not improving on current therapy then ER visit is prudent. Was any imaging done to assess for a kidney stone as well?
