Variable: When your sugar is very high and not coming down with your conventional therapy. There is no absolute number, it depends upon your medical provider and their skill and managing it as an outpatient. Some would have you do that with a sugar as low as 350-400, others can manage glucoses much higher without a hospital visit. Depends upon the doctor and the situation and patient.

Blood sugars: Do not go too low unless you are on diabetes medication or have end stage cancer, kidney or liver disease, etc. Your body protects you from low glucose. You are probably overweight and Insulin resistant, and feeling a rapid return to normal after a meal. If your sugar is actually too high, then you have diabetes and need to see your doctor.
