Epic quest: That dose level is higher than normal (5mg-15mg) and usually only given to patients w/ a tolerance. The Dr will need to 'know' you and your Hx - and trust you. Keep searching. I hope this starts a response thread. Make calls and if denied, ask for a link to one willing to evaluate you for that dose.
