Topamax (topiramate) ok, but....: The treatment of Chiari malformation and its symptoms can be very difficult. Some doctors will use Topamax (topiramate) in an attempt to help with associated headaches and fluctuations in intracranial pressure. However, it is not a great medication for that purpose. Diamox (aka acetazolamide) can sometimes be more beneficial, but has some side effects as well. Find a doc who sees Chiari patients.

Chiari: symptoms sometimes improve with use of other mild carbonic anhydrase inhibitors like Zonegran or Diamox (acetazolamide). Reducing CSF production does not always help though.
