It's a definition: If a fetus is living, then an abortion hasn't happened. If a fetus dies on its own (naturally happens 15-20% of the time), it is a spontaneous abortion (a miscarriage). If a young fetus dies because the woman intentionally makes it die to stop her pregnancy, it is called a medical abortion (sometimes a surgical abortion). If an older fetus dies on its own, it is a stillbirth.
