C/S delivery/premi's: Few sets of triplets get past 36 weeks and essentially all are delivered c section. Beyond that, all the problems related to premature birth, premi lung disease, low blood sugar, etc. All should pre-plan delivery and have a plan for neonatal care should some or all of the problems arise.

Yes: A birth plan is helpful by simply having you and your partner discuss choices before the big day: are you interested in natural childbirth? If you want or need medication in labor, what choices are there? Do you want the flexibility to do things like walk in early labor? A birth plan is more about knowledge, and not about a written piece of paper. Be sure to communicate openly with your l&d nurse!

Talk with your doc: Many patients will form a birth plan. Many options are available online. I always like to go over the plan in detail in the office. That way we can discuss why or why not certain items are good idea or why doctors/hospitals do certain things. The key to any good doctor patient relationship is communication.

Uterus rupture/death: Failed trial of labor and repeat cesarean section is the most common risk when attempting vbac. It is in this subgroup of women with failed tolac and one prior kerr hysterotomy that the risk of uterine rupture is greatest reaching 15% (if oxytocin is used). When this dire labor complication occurs the fetus can die or sustain permanent brain injury. The mother is at risk for bleeding/infection.

The more the better.: Extrapolating from studies in twins that show that increased maternal gestational weight gain is associated with improved fetal/neonatal outcomes, i would say that there is no upper limit to weight gain with triplets. Just monitor your health closely with your maternal-fetal medicine specialist to assure you do not have diabetes mellitus and/or hypertension. Take Folic Acid 1-5 mg daily!
